SMS Consent (Optional)

SMS text message consent is a separate, optional opt-in on the registration form and is not required to register or volunteer. If you check the SMS consent box and provide a mobile number, you consent to receive text messages (SMS) from FeelFit Foundation. This program sends Account and volunteer-activity notifications only: registration and sign-in confirmations, reminders about volunteer shifts and events you're already scheduled for, and hour-logging reminders. We never invite you to new programs, events, trainings, or rewards selections by text, and we never send general marketing or fundraising appeals by text; those go by email only.

• Message and data rates may apply.
• Message frequency varies based on your volunteer activity and event registrations, and may be up to several messages per month.
• Opt-out information. You may opt out of text messages at any time by replying STOP to any message we send; you will receive one final confirmation and then no further messages from this program. Reply HELP to a text message for assistance.
• Your mobile number and SMS opt-in status are never shared or sold to any third party for marketing or promotional purposes. See our Privacy Policy for how we handle your information generally.

Liability Release & Assumption of Risk Waiver

In consideration of being permitted to participate in any way in FeelFit Foundation sports, recreation, fitness, and volunteer activities (the "Activities"), I, the undersigned, acknowledge, appreciate, and agree that:

1. Assumption of Risk. Participation in the Activities carries inherent risks including, without limitation, physical injury, illness, permanent disability, and death. I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation.
2. Fitness to Participate. I certify that I am physically fit, have not been advised otherwise by a qualified medical professional, and am responsible for disclosing any health considerations that may affect my safe participation.
3. Release of Liability. I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless the FeelFit Foundation, its officers, directors, employees, volunteers, and partner organizations ("Releasees") from any and all claims, demands, losses, or liability arising out of or related to any injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
4. Emergency Treatment. I authorize the Releasees to secure emergency medical treatment on my behalf if needed, and I accept financial responsibility for such treatment.

I have read this release of liability and assumption of risk agreement, fully understand its terms, and sign it freely and voluntarily.

Photo / Media Release

I grant the FeelFit Foundation and its authorized representatives the irrevocable right and permission to photograph, film, and record me during the Activities, and to use, reproduce, publish, and distribute such images, video, audio, name, and likeness in any media now known or later developed for promotional, educational, fundraising, and archival purposes, without compensation, inspection, or approval. I waive any right to royalties or other consideration arising from such use and release the Foundation from any claims related to it.
